Fees: Players- $35.00
Volunteers- $10.00
Joining the Miracle League is
easy.
To sign up to play
or be a volunteer download and print the forms.
Just fill out the
registration form, sign the medical release, sign the media release
(or have a responsible adult sign for you, if appropriate).
Mail the forms and $35 registration fee for players to;

Players and volunteers must
register. Only one person per form. Please make sure you
send in the correct release form.
One release form is for all players and volunteers who are under 18
years old.
The other release form is for adult volunteers (the registration
forms are identical).
